Purpose: We have the opportunity bless two local pastors with brand new chapels for their congregations to gather and worship in. We will also participate in sports ministry, vacation Bible school, and other outreach opportunities to serve and share the Gospel with local children and teens.

This form requires signatures of both parent or guardians in the presence of a Notary
